Annual Medicare Wellness Visits are free if you have had
Medicare Part B for longer than 12 months. The visit is performed by a trained Registered Nurse at the Family Medicine
Clinic of Sweetwater Memorial.
The service includes:
Health risk assessment during your visit. You will be asked some questions
before or during your visit. Your responses to those questions will help
you and your health professional get the most from your yearly Medicare
Wellness Visit.
Review of medical, family and social history.
Develop or update a list of current providers and prescriptions.
Height, weight, blood pressure, and other routine measurements.
Screening for cognitive impairment, fall risk and safety.
Personalized health guidance and recommendations for screenings you qualify
for under Medicare coverage.
A personalized Medicare Wellness Plan for the following year, outlining
the next steps in your wellness journey.
